Introduction
Park City was established as a silver mining community in the late 1860s. Over the next 50 years the silver boom brought thousands of people to the area, giving Park City a population of 10,000 at its height, with over 100 saloons and a thriving red-light district. In the 1930s mineral prices dropped due to the depression and caused Park City’s economy as well as its population to wane. By the mid-1960’s Park City became famous for its great snow and beautiful slopes and is now home to three world class ski resorts: Park City Mountain Resort, Deer Valley Resort, and The Canyons Resort as well as the state-of-the-art Utah Olympic Park. In 2002 Park City was a host city to the 2002 Winter Olympic Games. The Park City area hosted 26 of the 78 medal events at three Olympic venues during a two-week period in February 2002.
At its inception in the 1880s, the Park City Fire Department (PCFD) was a volunteer fire department that protected only 17 square miles. In 1984 the PCFD became the Park City Fire Service District (PCFSD), a full and part-time fire agency that provides fire protection and Emergency Medical Services to more than 100 square miles of western Summit County.
The PCFSD began managing the North Summit Ambulance Service on behalf of Summit County in 2001. North Summit Ambulance (NSA) provides Intermediate-EMT ambulance transport services to a 500-square-mile area in northern Summit County. The area is predominantly rural with large agricultural tracts of land, some gas and oil drilling locations, and a couple of recreational reservoirs. There are three distinct communities (Henefer, Coalville, and Wanship) that make up the approximate 3,000 residents of the area. Interstates, 80 and 84, are the primary traffic arteries through the response area, which generate a significant percentage of the approximate 250 annual EMS calls.

Billing Questions
1) How do I make sure my insurance gets billed first?
Park City Fire Service District (PCFSD) will invoice your insurance as a courtesy to you. If you do not have insurance information available at the time of treatment, the patient or responsible party should contact PCFSD at (435) 940-2511. The following information must be provided within 10 days of ambulance transport:
- Patient name, mailing address, phone number and date of birth
- Insurance company name, address and phone number
- Insurance type (medical, automobile, industrial, Medicare/Medicaid)
- Plan number
- Group number
- Insured dame (primary card holder and the patient’s name if different)
2) When should I expect to see an ambulance billing statement?
It will take approximately thirty days before you will receive an ambulance statement.
3) Who should I contact with questions regarding my statement?
Once you have received your statement, any questions should be directed to physician’s Billing Office at (888) 834-5032, ext. 5142
4) What if I do not have insurance?
Please contact the PCFSD at (435) 940-2511 to arrange a payment schedule. Persons making an honest effort to address the financial obligation and willing to make monthly payments will not be sent to collections.

Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Park City Fire Service District (Summit County/Park City Ambulance, dba “Park City Ambulance”) is required by law to maintain the privacy of certain confidential health care information (Protected Health Information or “PHI”) and to provide you with a notice of our legal duties and privacy practices with respect to your PHI.
Uses and Disclosures of PHI: Park City Ambulance may use PHI for the purposes of treatment, payment, and health care operations, in most cases without your written permission. Examples of our use of your PHI:
For Treatment: This includes obtaining verbal and written information about your medical condition and treatment from you as well as from others, such as doctors and nurses who give orders to allow us to provide treatment to you. We may also give verbal or written information about your PHI to other health care providers involved in your treatment.
For Payment: This includes any activities we must undertake in order to get reimbursed for the services we provide to you, including such things as submitting bills to insurance companies, making medical necessity determinations, and collecting outstanding accounts.
For Health Care Operations: This includes quality assurance activities, licensing, and training programs to ensure that our personnel meet our standards of care and follow established policies and procedures, as well as other management functions.
Use and Disclosure of PHI Without Your Authorization: In addition to that already stated above, Park City Ambulance is permitted to use PHI without your written authorization, or opportunity to object, in additional situations, and unless prohibited by a more stringent state law, including:
- For health care and legal compliance activities;
- To a family member, other relative, close personal friend, or other individual involved in your care if we obtain your verbal agreement to do so or if we give you an opportunity to object to such disclosure and you do not raise an objection, and in certain other circumstances where we are unable to obtain your agreement and believe the disclosure is in your best interests;
- To a public health authority in certain situations as required by law, such as to report abuse, neglect, or domestic violence;
- For health oversight activities, including audits, government investigations, inspections, or other related activities;
- For limited law enforcement activities, such as responding to a warrant, as well as military, national defense and security, and other special government functions;
- To avert a serious threat to the health and safety of a person or the public at large;
- For workers’ compensation purposes and in compliance with workers’ compensation laws;
- To coroners, medical examiners, and funeral directors for identifying a deceased person, determining cause of death, or carrying out their duties as authorized by law;
- If you are an organ donor, we may release information to facilitate organ donation and transplantation;
- Subject to strict oversight and approval, for research projects;
- We may also use or disclose health information about you in a way that does not personally identify you or reveal who you are.
Any other use or disclosure of PHI, other than those listed above, will only be made with your written authorization. You may revoke your authorization at any time, in writing, except to the extent that we have already used or disclosed medical information.
Patient Rights: As a patient, you have rights with respect to your PHI, including:
- The right to access, copy, or inspect your PHI. We will normally provide you with access to this information within 30 days of your written request. In limited circumstances, we may deny you access to your medical information, and you may appeal certain types of denials. If you wish to inspect and copy your medical information, you should contact our privacy officer;
- The right to amend your PHI. You have the right to ask us to amend written medical information that we may have about you. We will generally amend your information within 60 days of your request. We are permitted by law to deny your request to amend your medical information in certain circumstances, such as when we believe the information you have asked us to amend is correct and complete as written;
- The right to request an accounting. You may request an accounting from us of certain disclosures of your medical information that we have made in the six years prior to the date of your request. We are not required to give you an accounting of information we have used or disclosed for purposes of treatment, payment, or health care operations.
- The right to request that we restrict the uses and disclosures of your PHI. You have the right to request that we restrict how we use and disclose your medical information that we have about you. Park City Ambulance is not required to agree to any restrictions you request, but any restrictions agreed to by Park City Ambulance in writing are binding.
Revisions: Park City Ambulance reserves the right to change the terms of this Notice at any time. Changes will be effective immediately and will be promptly posted to our web site. You can also get a copy of the latest version of this Notice by contacting our privacy officer.
Your Legal Rights and Complaints: You also have the right to complain to us, or to the Secretary of the United States Department of Health and Human Services if you believe your privacy rights have been violated. You will not be retaliated against in any way for filing a complaint with us or the government. Should you have any questions, comments, or complaints, you may direct all inquiries to our privacy officer.
